    We coordinate with the attending physician and our medical team on certification, orders and records.

Is it time to consider hospice?

Hospice is for people whose physician certifies a life expectancy of six months or less if the illness runs its normal course, and who choose comfort-focused care. Tick what applies — this is general guidance, not a medical assessment.

Who can make a hospice referral?

Anyone can start the conversation — physicians and nurse practitioners, hospital case managers and discharge planners, nursing facility and assisted-living staff, home-health agencies, and patients or family members themselves. Admission to hospice requires physician certification, and we coordinate that with the patient's doctor.

What information should I have ready?

Whatever you have is enough to start. Helpful details include:

  • Patient name, date of birth and current location
  • Primary diagnosis and relevant history
  • Attending physician's name and contact
  • Insurance information (Medicare, Medicaid or other)
  • Family or caregiver contact

If something is missing, call anyway — our intake team will help fill in the gaps.

Does the patient have to stop seeing their doctor?

No. Patients can keep their own doctor. Their physician can stay involved and work alongside our hospice team and medical director on the plan of care.

How is hospice paid for?

As a Medicare provider, people aged 65+ are eligible to be covered for our hospice services at no cost. Many other insurance plans also include a hospice benefit — call us and we'll help check coverage.

Where is care provided?

Wherever the patient lives — their home, a nursing facility or an assisted-living community. When symptoms can't be managed at home, General Inpatient Care is available, and Inpatient Respite Care gives caregivers a short break.

What if I'm not sure the patient is eligible yet?

Not sure? Call us — we'll help determine eligibility. It's always fine to ask early; a conversation doesn't commit anyone to anything.
